Well, it's a bit of an inside joke, but I'll try to explain it.
The text on the left says 'pssst, girl, post feet!' in very crappy imageboard Dutch. What Katja says translates to something like 'cancer mongol', I'm not sure if krebsmongo is even a word in German, I think it isn't, but that's part of the joke.

Not sure if the Germans actually say it. It's pretty much a literal translation of 'kankermongool'.
